I (W. Wertelecki, M.D.) see an important example how simple visual examination can be nearly diagnostic. The “virtual” becomes near certainty with information from the family history. The family history is an important essential diagnostic sign in many clinical disorders, including Neurofibromatosis type 1 (NF1). Note more than one oval with smooth borders melanotic spots with an oblique axis. These are characteristics of café-au-lait spots. In combination with a positive family history is virtually diagnostic – “the clinical moral” is to examine every first degree of relative. The posting of more images (some dramatic) of such relatives are planned.
