I (W. Wertelecki, M.D.) see a palate perforation – not a cleft. Two main possibilities arise, as usual – either this sign is isolated or is associated with other concurrent signs. Open your eyes and it may help to synthesize a hypothesis of cause and diagnosis (see related gallery).
PERSPECTIVE: The two main possibilities are cocaine use or syphilis.
